Through our health education on chronic disease self-management initiative, a good number of the older persons have developed the ability to promote and maintain their health as well as manage illness and disability.. This self-care practice is a vital aspect to ageing in place and we plan to share with others. Working and collaborating with individuals and organizations on this platform that shares similar vision and mission is a great way to achieve our goals
